Do I need to know the exact trade or solution?
No. Describe what you can see and what you want addressed. Haven reviews the next step and whether a qualified specialist is needed. A request is not a diagnosis, confirmed appointment or authorization to begin work.
Who handles the work and agreement?
Haven coordinates the request and follow-up. Before approval, the proposal identifies the service provider, scope, agreement, charges and responsibility. Independent-provider work is contracted directly with that provider; any Haven-direct work has its own agreement with Haven Care Network, LLC.
